System is not under memory pressure, CPU usage is none (non production pre-deployment)

Problem: Simple file operations like cp, rsync, and even direct dd (with iflag=direct oflag=direct) to/from XFS filesystems are extremely slow (as low as 2 MB/s), even when copying small files (e.g., 50 MB). However, when testing with fio, I get excellent throughput — up to 3 GB/s read and 1.8 GB/s write, both to the mounted filesystem and directly to the device (e.g., /dev/mapper/mpathc1).

Even cat file to /dev/null shows 3.2 MB/s while fio with same file shows 2-3 GB/s.

What I’ve tried:
Disabled caching by using iflag=direct/oflag=direct
Switched between sync and async mount options (no impact)
Tested I/O accounting and cgroups (working fine)
Verified ALUA and multipath priorities (properly balanced, no path errors)
Used nice, ionice, systemd-run --scope with IOWeight, CPUWeight
Analyzed with strace, which shows normal copy loop (read/write syscalls)
Set zero to /proc/sys/vm/dirty_ratio and dirty_background_ratio
Ensured proper multipath.conf configuration (round-robin, no_path_retry, etc.)

Notable Behavior:
All read/write commands to the device (e.g., dd, cat, cp, rsync) behave unusably slow.
fio works perfectly — both file-based and raw-device-based tests.
The issue does not appear to be SAN-related or bandwidth-constrained.
I/O statistics (iostat, vmstat, iotop) show minimal activity during these slow operations.
No filesystem corruption — XFS mount is clean, with no logs or dmesg errors. Fresh system.

Question: What could explain the massive performance discrepancy between fio and basic file operations like cp or rsync?
