Research Proposal Endonasal Correction of Caudal Septal Deviation: A Systematic Review of Surgical Techniques and Outcomes Background Deviation of the caudal nasal septum is a common cause of nasal obstruction and may also contribute to nasal tip asymmetry and external nasal deviation. Because the caudal septum forms the anterior component of the septal L-strut, deformity in this region can significantly affect both nasal airflow and structural nasal support. Correction of caudal septal deviation is technically challenging. Numerous surgical techniques have been described, including: swinging-door repositioning of the caudal septum modified swinging-door techniques with fixation to the anterior nasal spine cartilage scoring or wedge resection caudal septal batten graft stabilization extracorporeal septoplasty. Despite the variety of approaches, the literature remains fragmented and consists largely of small observational series and technique reports. Many studies evaluate general septoplasty outcomes but do not specifically focus on correction of caudal septal deformities, and there is limited synthesis of outcomes across different surgical techniques. Furthermore, there is no widely adopted framework for organizing the available techniques according to the type of caudal septal deformity being treated, which makes comparison of results across studies difficult. A systematic review of the available literature could provide a structured synthesis of current surgical approaches and reported outcomes. Objectives The primary objective of this study is to systematically review the literature describing endonasal surgical correction of caudal septal deviation. Specific aims include: Identify surgical techniques described for endonasal correction of caudal septal deviation. Summarize functional outcomes reported following these procedures. Evaluate complication and revision rates associated with different surgical techniques. Develop a practical surgical classification system based on the techniques described in the literature. Propose a treatment algorithm that links commonly reported deformity patterns with the surgical approaches used to correct them. Estimate pooled functional outcomes, when feasible, using reported measures such as the NOSE Scale. These additional elements—classification, algorithm development, and pooled outcome analysis—are intended to enhance the clinical relevance of the review and improve its value for surgical decision-making. Methods Study Design This study will be conducted as a systematic review following PRISMA guidelines. Data Sources A comprehensive literature search will be performed in: PubMed Embase Scopus Search terms will include combinations of keywords related to: caudal septum septal deviation septoplasty endonasal or intranasal surgical techniques. Reference lists of included articles will also be reviewed to identify additional relevant studies. Eligibility Criteria Inclusion criteria studies describing surgical correction of caudal septal deviation endonasal or intranasal septoplasty techniques studies reporting postoperative clinical outcomes. Exclusion criteria cadaveric or purely anatomical studies technical descriptions without clinical outcome data rhinoplasty studies without septal correction. Study Selection Two independent reviewers will screen titles and abstracts for eligibility. Full texts of potentially eligible studies will then be reviewed. Disagreements between reviewers will be resolved through discussion. Data Extraction The following variables will be extracted: study characteristics patient demographics description of caudal septal deviation surgical technique used use of cartilage grafts or structural reinforcement functional outcome measures (e.g., NOSE Scale) complication rates revision surgery rates follow-up duration. Data Analysis Results will be summarized descriptively. Surgical techniques will be grouped into categories including: repositioning techniques cartilage modification techniques graft-based stabilization techniques extracorporeal reconstruction techniques. When sufficient data are available, pooled estimates of functional outcomes will be calculated. Additionally, the literature will be used to develop: a classification system for caudal septal surgical correction techniques, and a treatment algorithm illustrating commonly reported surgical strategies. Expected Impact This systematic review will provide a comprehensive synthesis of the literature on endonasal correction of caudal septal deviation and summarize outcomes associated with commonly used surgical techniques.
